What the form is

Form บต.37 (Form WP. 37) is the “Receipt Confirmation Form for the Work Duration Extension Request for a Foreigner Performing the Works which can be Characterized as Necessary, Urgent, or Ad-hoc pursuant to Section 61” of the Department of Employment (กรมการจัดหางาน), Ministry of Labour of Thailand.

The registrar (นายทะเบียน) issues it when a foreigner who has already notified urgent work under Section 61 (BT. 34, receipt BT. 36) asks with form BT. 35 to extend the work period. The Department publishes it as a fillable PDF, so the data can be typed in advance.

Who fills it in and where it goes

  • the data are the same as in BT. 35 and BT. 36: the foreigner, the number and date of the BT. 36 receipt, the employer or hirer, the place of work and the extension period;
  • it is handed in together with BT. 35 to the registrar of the employment office that accepted the notification; ask the office whether a pre-filled receipt is needed;
  • the receipt number and date, the signature, name and position of the registrar are filled in by the office;
  • no fee is printed on the form — check with the office.

What goes on the form

  • the foreigner's name, nationality and passport (or document in lieu of passport) number;
  • the date of the original notification and the work commenced;
  • the employer or hirer and the number and date of the BT. 36 receipt;
  • the work to be extended and the address and phone of the place of work;
  • the extension period in days with start and end dates (no more than 15 days under Section 61).

How to fill it in with FoxDocs

What is form BT. 37?
