What this document is
“แบบหนังสือรับรองวุฒิการศึกษาและประสบการณ์การทำงาน” (Education and Job Experience Certification Form) is a form of the Thai Department of Employment (กรมการจัดหางาน, DOE). In it, the Thai employer that wants to hire a foreigner certifies that the foreigner has the education and job experience required for the position stated in the work permit application.
The form is signed by the foreign applicant, the employer and two witnesses. Its text states that the signatories accept prosecution under section 137 of the Thai Criminal Code (false statement to an official) if any information is untrue.
Who signs it
- the applicant — the foreigner applying for a work permit;
- the employer — the person signing for the company, partnership or shop;
- two witnesses.
What the form asks for
- the applicant's title (Mr./Mrs./Miss) and name;
- the name and position of the employer's signatory and the name of the company, partnership or shop;
- the position the foreigner is hired for;
- education: highest qualification, field and year;
- job experience: up to three jobs — position, employer and years;
- signatures — by hand.
What to prepare
- the applicant's passport;
- the diploma and other education documents;
- documents confirming previous jobs, if available;
- the company registration certificate showing who may sign for the company.
