What this document is

This is the power of attorney form (หนังสือมอบอำนาจ, form T.D. 21) of the Thai Department of Lands (กรมที่ดิน) with the printed subject “ขอแก้คำนำหน้านาม และสกุล ตามสามี” — correction of title and surname to the husband’s. With it the principal authorises another person (agent) to file this application at the Land Office and to give statements to the officer.

The printed powers state that the principal has married, refer to the attached marriage certificate and change the name from “น.ส. …” (Miss, former name) to “นาง …” (Mrs, name with the husband’s surname). The header of the form describes a plot with a โฉนด title deed.
