What this form is

“ข้อมูลบุคคล / Personal Information” is a Thai Immigration Bureau form (แบบบันทึกข้อมูลบุคคลสำหรับการขอมีถิ่นที่อยู่) in which a foreigner applying for permanent residence gives details about themselves and their family. It is filed together with the main TM.9 application and the other documents on the Immigration Bureau checklist.

The form (2568 edition) has two pages and is bilingual (Thai and English). It has 16 items: name and sex, date and place of birth, present and original nationality, passport, date of first entry, address and phone in Thailand, occupation, place of work and monthly income, parents, husband or wife and marriage, children (up to four), education, height, weight and marks, other information, signature and date.

Who fills it in and where it is filed

  • The permanent residence applicant — the foreigner filing TM.9 — fills it in.
  • It is filed with the Immigration Bureau as part of the permanent residence package, at the office and during the period named in the annual announcement.
  • The fee and the full document list depend on the application category and year — check them with the Immigration Bureau.

Documents usually filed with it

  • the TM.9 application and this sheet signed by the applicant;
  • the passport and copies of the pages with visas, entry stamps and extensions;
  • documents for your category: work permit and employer documents, marriage certificate and spouse’s documents, children’s birth certificates, diplomas;
  • other forms and certificates listed in the Immigration Bureau announcement.

The exact checklist changes — check it with the Immigration Bureau before filing.

Tips for filling it in

  • Names in Latin letters as in the passport; everything else in English.
  • Dates are written as DD/MM/YYYY.
  • Item 6 asks for the date of your FIRST entry since which you have lived in Thailand, not the latest entry.
  • If a parent has died, write DECEASED in the address box.
  • Income, education and family details must match the documents you attach.
  • The signature is handwritten.

Frequently asked questions

Why is this sheet needed if there is TM.9?

TM.9 is the permanent residence application itself. The Personal Information sheet adds details about the applicant’s family, work, income, education and appearance and is filed in the same package.

Who signs it?

The applicant, by hand, with the date under the signature.

Which entry date goes into item 6?

The date of your first entry since which you have lived in Thailand continuously, not the date of your latest entry.

What if a parent has died?

Write DECEASED in the parent’s address box; name and nationality are filled in as usual.

I have more than four children — what should I do?

The form has room for four children. The others can be listed briefly in item 16 “Other information” or on a separate sheet — check with the Immigration Bureau.

Do I fill in the spouse section if I am not married?

No, item 12 is left empty.
