What this document is

TM.20 (ตม.20, “คำร้องขอรับใบแทนใบสำคัญถิ่นที่อยู่”) is the application of a permanent resident of Thailand for a substitute of the Certificate of Residence (ใบสำคัญถิ่นที่อยู่) when the original has been lost, damaged or destroyed.

  • nationality, race (เชื้อชาติ), occupation;
  • place of birth (town, district, province, country) and date of birth;
  • address in Thailand: house number, road, tambon, amphoe, province;
  • date of entering Thailand, residence permit number and date;
  • details of the certificate previously held: number, date, place, district and province of issue;
  • what happened to the original (lost, damaged, destroyed);
  • dates on the form: day, month and Buddhist Era year (พ.ศ.).

What to attach

  • two 4 × 6 cm photographs — required by the text of the form;
  • passport and copies;
  • if the original was lost — usually a police report on the loss; if damaged — the damaged certificate; check with the office.

The exact document list, fees and processing time are not stated on the form — check with the office before you go. The signature or right thumb print is added by hand; page 2 (height, scar, substitute number, official record, order) is completed by the Bureau.
