What this document is

TM.95 (ตม.95) is the form a holder of the Long-Term Resident (LTR) visa uses to tell the Thai Immigration Bureau that they have stayed in Thailand for 1 year and to confirm their present address. The form is bilingual (Thai and English), has one page and is an attachment to Immigration Bureau Order no. 150/2565 dated 22 August 2022.

It is not an extension of stay: the receipt part of the form says so explicitly. The lower part (“for official use only”) is completed by the officer and serves as the receipt with the date of your next notification; keep it in your passport.

Who files it and where

  • a foreigner holding an LTR visa who stays in Thailand for more than 1 year;
  • according to the form, the address must be notified to immigration every year;
  • where to file (the immigration office for your area or the One Stop Service Center for LTR holders) and whether other channels are possible — check with the office or the LTR programme website;

What the form asks for

  • the immigration office and date (year in the Buddhist calendar, พ.ศ.);
  • full name in Latin block letters and nationality;
  • date of the latest entry, flight or vehicle, passport number and arrival card number;
  • present address in Thailand: house number, lane/road, sub-district, district, province, phone;
  • applicant's signature — by hand.

Documents usually brought along

  • passport with the LTR visa;
  • copies of the photo page, the visa and the latest entry stamp;
  • the receipt of the previous notification, if any;
  • the exact list depends on the office — check in advance.

What is the TM.95 form for?

How is TM.95 different from TM.47?
