What this document is
The Power of Attorney (หนังสือมอบอำนาจ) is a bilingual (Thai and English) one-page form of the Thai Ministry of Foreign Affairs. With it, a parent (the grantor) authorizes another person (the authorized representative) to submit the Thai passport application for the parent’s child and to take related actions until completion.
The grantor accepts responsibility for what the representative does under this power of attorney as if done personally. Although catalogues sometimes call it a “general” power of attorney, the text of this form covers only the child’s Thai passport application.
Who files it
- a father or mother of a Thai national child who cannot attend the passport application in person;
- the representative is the person who will go to the passport office or embassy instead of the parent.
Whether a power of attorney is accepted in your particular case (for example, the child’s age or whether the other parent must also consent) is decided by the passport office or embassy — check with them in advance.
What the form asks for
- place and date of writing (Buddhist Era year, พ.ศ.);
- for the grantor and for the representative: title (Mr./Mrs./Miss — the others are struck out), first name, family name, age, nationality, race (เชื้อชาติ), current address, Thai ID card or passport number, place of issue, dates of issue and expiry, contact phone number;
- the child’s title (under 15: ด.ช./ด.ญ.) and full name.
The grantor, the representative and two witnesses sign by hand. The certification block (“I hereby certify…”), the official’s signature, position and seal are completed by the office.
Where to sign it and what to bring
- according to the remark on the form, the grantor must sign before an official at a district office (สำนักงานเขต/อำเภอ) in Thailand or at a Royal Thai Embassy / Consulate-General abroad;
- bring your passport or Thai ID card; ask the office whether copies of your and the representative’s identity documents are required.
The form does not list attachments, fees or processing times — check with the Department of Consular Affairs passport office or the embassy. For the child’s passport the office may also require a parental Letter of Consent, which is a separate document.
