What this document is

หนังสือมอบอำนาจ (Power of Attorney) is a simple power of attorney: the grantor (ผู้มอบอำนาจ) authorises one or two attorneys (ผู้รับมอบอำนาจ) to carry out specific acts and sign documents on the grantor's behalf.

It is a widely used private bilingual template (Thai text with English field captions), not a form issued by a specific government agency. Many offices accept a free-form power of attorney, but some have their own forms — check in advance.

When it fits and when it does not

  • it suits everyday errands: submitting or collecting documents, obtaining a certificate, representing the grantor at a company, bank or government office — if that office accepts a free-form power of attorney;
  • for registering land and house transactions at the Land Office, the Department of Lands usually requires its own power of attorney form ท.ด.21 (อ.ช.21 for condominiums); this template is generally not used there;
  • if the power of attorney is signed outside Thailand, the receiving office may require notarisation and/or legalisation at a Thai embassy — check the requirements.

What the form asks for

  • place and date (year in the Buddhist Era, พ.ศ.);
  • the grantor's title and name;
  • title, name, position and workplace (name, phone, address) of one or two attorneys;
  • the powers — what exactly the attorney may do (up to three lines);
  • signatures of the grantor, attorneys and two witnesses — by hand; company seal if the grantor is a company.

What is usually attached

  • copies of the passport or Thai ID card of the grantor and the attorney, signed by their holders;
  • documents for the errand itself (for example a passport copy with stamps for immigration, or the property documents);
  • a duty stamp (อากรแสตมป์) in the box at the top right — the template shows 10 baht; the required amount depends on the scope of the powers, check with the office.

The exact list of documents and fees depend on the office where the power of attorney is used — check in advance.

Is this an official government form?

No. It is a widely used private bilingual template. Many offices accept a free-form power of attorney, but some have their own forms — check in advance.
