What this document is

The “Application for a Letter of Residence Information in the Kingdom of Thailand” (แบบคำขอหนังสือรับรองข้อมูลการแจ้งที่พักอาศัยในราชอาณาจักรไทย) is a foreigner's request to the Immigration Bureau for a letter confirming the address notified to immigration.

This is the 2026 edition (B.E. 2569) published on the Immigration Bureau website. Unlike the older Bangkok form, no province is pre-printed — the form is used nationwide.

It has two pages: “ผนวก ก” is the application the foreigner fills in; “ผนวก ข” is the template of the letter issued by immigration.

What the letter is used for

  • obtaining a Thai driving licence (car or motorcycle) at the Department of Land Transport;
  • buying or selling a car or motorcycle;
  • opening a bank account — the form asks for the bank and branch.

The form lists no other purposes. If you need the letter for something else, ask immigration which document applies. The letter template states it is valid for 90 days from the date of issue.

What the application asks for

  • place and date of writing (the year in the Thai date line is in the Buddhist Era);
  • name with title (MR., MRS., MISS), nationality, date of birth;
  • passport number and TM.6 arrival card number, if one was issued;
  • date of the latest entry and the date you are permitted to stay until (DD/MM/YYYY);
  • address in Thailand: house no., name of residence, moo, soi, road, sub-district, district, province, zip code and phone;
  • the purpose and, for a bank account, the bank and branch;
  • consent for immigration to disclose your information to the named institution;
  • signature — by hand.

Documents usually brought along

  • passport and copies of the photo page, visa and latest entry or extension stamp;
  • the TM30 receipt for your current address — the letter confirms an address already notified to immigration;
  • proof of address (lease, owner's documents) if the office asks;
  • photographs, if the office requires them.

The document list, any fee and processing time depend on the office — check with your immigration office. The form does not state them.

How to submit

  1. Fill in the application (ผนวก ก) in Latin letters and sign it.
  2. Attach passport copies and the TM30 receipt.
  3. Submit it at the immigration office responsible for your address; check opening hours in advance.
  4. Collect the letter and take it to the Department of Land Transport or the bank.
